More than 60 days since the Chinese Lunar New Year’s Eve, Wuhan Hanyang Hospital has undergone three transformations from COVID-19 non-designated hospital to designated hospital and then to clean hospital. And through all these transformations, Hanyang Hospital fully performed the advantages of private hospital such as fast action, quick decision-making, high efficiency and human resources, and provided outpatient and emergency medical services to 80,000 non-COVID-19 patients in the first quarter of this year.

Fully committed at the beginning of the outbreak

Zhang Fu, Chairman of the Board of Wuhan Hanyang Hospital, introduced the situation during early stages of the epidemic. In order to satisfy people's need of medical treatment service and in the meantime to protect medical staff themselves, Hanyang Hospital has separated a building as a non-COVID-19 ward and other areas of the hospital providing normal medical services on January 25, which operates effectively block the risk of nosocomial infection.

In the early days of the outbreak, the number of patients increased rapidly. In order to ensure the treatment of patients, emergency medical director Zuo Xiaobing adjusted the way doctors on duty. The day shift was extended from 8 a.m. to 23 am., for a     total of 15 hours. During this period, Zuo and his colleagues in the department of       emergency medicine engaged 700 patients almost every 24 hours, of whom about 50 were in critical condition. In the day with the most patients, 7 nurses received 700    injections in 24 hours.

As COVID-19 designated hospital, trying to satisfy daily medical needs for non- COVID-19 patients

On February 2, according to the overall arrangement of fighting against the epidemic in Wuhan, Hanyang Hospital was transformed into a designated hospital for treatment of COVID-19. On the day of the transformation, Hanyang Hospital completed the transfer of 200 patients and more than 20 departments combined operation.

In consideration of the daily medical service needs of the people, Hanyang Hospital insisted on opening general outpatient and emergency services in the isolation area and accepting non-COVID-19 patients according to its own conditions. On February 24, Lao Jin, who was fighting in the front line of the community, accidentally got a fish bone stuck in his throat. Due to the difficulty in seeking medical treatment during the epidemic, he had been hospitalized in Hanyang Hospital for a week. His family cried, "If the fish bone is not taken out again, he will starve to death." Guo Changkai, Department Director of otolaryngology in Hanyang Hospital, immediately arranged the operation and removed a 3-cm-long fish bone from his pharynx.

Wang Hui, who works in the emergency service center, said that during the operation of designated hospitals, the life of the emergency medical staff and drivers were very tough. During the peak period of the epidemic, all hospitals were in a state of saturation, and it took up to 12 hours to find a hospital to treat a patient. Due to the high infectivity of COVID-19, patients cannot be accompanied by their family members when they go to the hospital to go through the admission procedures and conduct accompanying examinations. The work that would have been done by their families was also done by these 120 emergency medical staff.

Transform to clean hospital at the first time

Xie Bin, Vice President of Hanyang Hospital, said that in the situation of the aggravation of chronic patients' condition and the increasing number of emergency patients, in order to rationalize the relationship as soon as possible, to treat non-COVID-19 patients and meet the medical needs of the people, Hanyang Hospital took the lead in transforming into a clean hospital on February 27, thus becoming the only large-scale general hospital in Hanyang district to receive ordinary patients. For this reason, Hanyang Hospital adjusted the management system, broke the division of daily departments, unified allocation and management of all hospital wards, and solved the problems of difficult admission and difficult medical treatment.

Yin Xiaoxin, Vice President of Hanyang Hospital, introduced that in order to ensure the cleanliness of the hospital after the transformation, Hanyang Hospital first carried out the thorough elimination of meticulous to the ceiling and desktop. After that, temperature detection points were set up at both entrances and exits of the hospital for screening. In addition, a second temperature measurement and a history of exposure were performed in the outpatient and emergency department before the patient was admitted to the clinic. At the same time, the consultation room has been adjusted according to the current situation, implementing that one doctor receiving one patient in one consultation room. The patients requiring hospitalization should be checked for their blood, chest CT, nucleic acid and serum antibody, and tried not to be escorted. If the patients do have to be escorted, their family members or escort personnel also have to do the COVID-19 screening. When screening showed normal, the patient and family members can be admitted to hospital. And one patient in one ward was guaranteed.

Furthermore, all departments have prepared 3-5 extra rooms, one room for one person. Because the general buffer period is about 3 days, the patients suspected of COVID-19 must enter the buffer room first. After screening, they could transfer to the ordinary ward with other patients.

On this basis, the patients will get treated according to their degrees of illness. For some patients with mild symptoms, they should set up a doctor's appointment as far as possible. For the emergency and severe patients, green channel is guaranteed all day long.

At present, with the outbreak under further control, Hanyang Hospital and all walks of life in Wuhan city are gradually restored to normal order. Xie Bin introduces except emergency operation patients that has mentioned above, the patient of average treatment also can be admitted to hospital now. What’s more, the fever outpatient department also returns to normal outpatient service at present.
